VON Connect, welcome to Ontario!

We are thrilled to share that VON Connect, powered by AlayaCare, has officially launched across our Southwestern Ontario region, marking our first launch in Ontario since the North Bay and Porcupine beta launch and largest single-site rollout to date. On July 28th, after months of planning, training, and preparation, our London, Middlesex Elgin, Grey Bruce, and Strathroy teams have successfully gone live with our new electronic health record (EHR) system.

This milestone represents the first of many as we continue our digital transformation across Ontario, with upcoming launches planned for our Hamilton Niagara Haldimand Brant (HNHB) region and Erie St. Clair (ESC) sites. 

VON Connect is more than just a technology upgrade, it is a transformative shift in how we deliver care. With EHRs now accessible on mobile phones and tablets, our care teams are transitioning away from paper-based documentation and embracing real-time, centralized access to client health information. This ensures greater accuracy, improved communication, and enhanced quality of care at every step.

The four launch sites are home to a variety of essential programs, including nursing and home support services, as well as community-based initiatives like Meals on Wheels, CAPABLE, and Adult Day Programs. With VON Connect, staff in all areas are better equipped to support clients and their caregivers with timely, personalized, and coordinated care.

What this launch means for our clients and teams:

  • Team members can now access up-to-date client information across locations, improving the consistency and quality of care.

  • Real-time documentation supports faster decision-making and stronger coordination among care providers.

  • Administrative tasks are streamlined, reducing paperwork and allowing more time for direct client care.

  • Clients will experience more personalized and proactive support, helping them remain safely at home and in their communities.

  • A digital-first approach reduces paper clutter in clients' homes and lightens the environmental footprint.

  • The system strengthens our ability to track outcomes, identify health disparities, and continuously improve care through data insights.

This achievement was made possible by the dedication and teamwork of our incredible staff across Southwestern Ontario. We extend our heartfelt thanks to everyone involved in making this launch a success.
