+/- Blood Pressure Clinics in North Perth

VON’s Wellness for Seniors initiative introduces Cardiovascular Health Awareness Program (CHAP).  It is a community-based program that brings together local family physicians, pharmacies, allied health professionals, public health representatives, volunteers, and community organizations to work as partners to promote and actively participate in the prevention and management of cardiovascular disease and stroke.

The goal of CHAP is to implement a peer health educator program of cardiovascular health awareness, including blood pressure monitoring in communities across Ontario and elsewhere.

CHAP is made available free of charge to older adults in the community.  It offers opportunities for enhanced multiple blood pressure monitoring and other health promotion, prevention and management activities addressing modifiable risk factors for cardiovascular disease and stroke.  

Location & Days Clinics are Open

1st Monday of the Month - VON Office, Listowel - 9:30 a.m. to 12:30 p.m.

4th Tuesday of the Month - Zehrs Pharmacy, Listowel - 9:30 a.m. to 11:30 a.m.

3rd Thursday of the Month - Shoppers Drug Mart, Listowel - 9:30 a.m. to 12:30 p.m.

+/- Visiting Nursing

Visiting Nursing - provided by small self-directed nursing teams of RNs and RPNs.

Shift Nursing - A minumum of 3 hours to a maximum of 24 hours, provided by RNs and RPNs.

DAYS OF SERVICE:  7 days a week

HOURS OF SERVICE:  24 hours a day {after 5:00 p.m. visiting nurse "On Call"}

CLIENT DEMOGRAPHICS:  All age groups - infants to seniors

SERVICE COMPONENTS:

* Assessments / Health Teaching
* Case Management / Coordination
* Post-operative Care
* Care of the physically challenged and chronically ill 
* Rehabilitation Nursing
* Mother and Baby Care
* Palliative Care
* Health Counselling
* Continence Management
* Ostomy / Wound Management
* Management of Medically Fragile Children
* Management of Acquired Brain Injury Clients

COSTS:  Private rates upon request (for financial assistance, a sliding fee scale is available)
