Locations: 1st Tuesday of every month, Travelling Dining
1st Wednesday of every month at the Derry Street Apartments
2nd Wednesday of every month at the Perth Meadows, Listowel
3rd Wednesday of every month at the Optimist Hall, Monkton
4th Wednesday of every month at "The Village" Clubhouse, Listowel

Hours of Service:  12:00 to 2:00 p.m.                               

Client Demographics:
* Seniors and disabled adults who are living in the community who would benefit from the nutritional and social aspects of the Congregate Dining service.

Service Components:
* a hot, nutritious, full course meal
* interesting social activities such as cards, games, entertainment, etc. or informative sessions relating to issues that may concern the participants
* transportation to and from the service can be arranged, if required

Transportation:
* participant, family and/or friends
* volunteer transportation can be arranged, upon request

User Fee:  $10.00 per person

+/- Home Maintenace Referral Service

LOCATION OF SERVICE DELIVERY: Northern part of Perth County including Listowel, Atwood, Gowanstown, Molesworth and surrounding rural areas

CLIENT DEMOGRAPHICS:  Serving seniors or disabled adults living in the community who require assistance with light or heavy home maintenance jobs that are beyond their capabilities.

SERVICE COMPONENTS:  The Service will provide a linkage between workers who have been through a screening process for security purposes, and consumers who require home maintenance, seasonal cleaning and/or minor home repairs.

Consumers call the North Perth Community Support Services Office; they are briefed about the service and then are given the names and telephone numbers of three (3) appropriate workers.  Consumers then contact and arrange for service.

USER FEE:  There is no cost for the client to receive referral names and telephone numbers.

Clients are responsible to pay a mutually agreed upon fee directly to the worker.

+/- North Perth Meals on Wheels

Location:  Eligible individuals residing in the Municipality of North Perth

Hours of Service:  Hot meals are delivered to clients by volunteers, Monday to Friday (except Statutory Holidays) between 11:45 a.m. and 12:30 p.m., within town limits of Listowel
Frozen meal delivery is available on Tuesday mornings, or arrangements can be made for pick-up during regular business hours at the VON North Perth Community Support Services office..

Client Demographics:  Seniors, adults with a disability, persons convalescing from illness or surgery, and person unable to care for their own nutritional needs.

Service Components:
* The purpose of the Meals on Wheels program is to provide hot or frozen nutritious meals
* Meals are delivered to client's homes by volunteers
* The program provides and promotes good nutrition
* Volunteer involvement provides regular social contact and a health and safety check
* Special diet requirements can be accommodated
* Referrals are made to other organizations when appropriate

User Fee:
 
Hot meals are $7.50 each - fees billed monthly
Frozen meals prepared by "Apetito" are $5.50 per entrée and $1.50 for soup or dessert - fees are payable at time of delivery/pick-up

+/- Palliative Care Volunteer Program

  

LOCATION OF SERVICE DELIVERY:  South Huron County, including Grand Bend, Bayfield, Zurich, Exeter and surrounding rural areas

HOURS OF SERVICE:  Program Coordinator is available 30 hours weekly.  Volunteers available as required

PATIENT DEMOGRAPHICS:   Palliative patients and bereaved families/friends in their homes, in hospitals or Long-Term Care facilites.

SERVICE COMPONENTS:
* Volunteers are matched with clients on a one-to-one basis 
*Volunteers offer support and friendship with regular visitsurage client interests/activities, planned outings and relief for caregivers
* Bereavement Consultant offers bereavement follow-up visits, support groups, resources, books, videos, and pamphlets

*USER FEES:  There is no cost to clients

Listowel:  Parkview Gardens - Legion Hall - Wednesdays: 10:30 a.m. - 11:30 a.m.

                  The Village Club House - Fridays:  10:30 a.m. - 11:30 a.m.

                  Perth Meadows - Apartment Complex - Tuesdays: 10:45 a.m. - 11:45 a.m. (Low mobility / Seated exercise)

Atwood:  Atwood Presbyterian Church  - Mondays 10:00 a.m. - 11:00 a.m.

Monkton:  Monkton Optimist Hall - Wednesday 9:00 a.m. - 10:00 a.m.

Sebringville:  Sebringville Community Centre - Fridays 9:00 a.m. - 10:00 a.m.

Length of Classes:  45 minutes to 1 hour classes

Client Demographics:
* seniors and older adults who value the benefits of exercise and wish to maintain and/or improve their own fitness level

Service Components:
* monthly educational component
* warm-up, range of motion, balance, cardiovascular, muscle-building, stretching, cool-down and relaxation exercises

Transportation:
If transportation is required the Program Coordinator will attempt to introduce / match non-drivers with drivers

User Fee:  The cost of the Program is $2.00 per person, per class.

+/- SMART In-Home Exercise Program

(Seniors Maintaining Active Roles Together)

Hours of Service:  As agreed upon by the in-home exercise volunteer and the client

Client Demographics:
* individuals who are 55 years of age or older and a resident of North
   Perth; who are home-bound, or have limited access to the community 
   due to physical limitations, problems with transportation or cultural
   barriers; and would like to enhance their level of fitness in the comfort of
   their own home
* isolated seniors recovering from a health crisis who would benefit from
   and are willing to participate in a gentle exercise program

Service Components:
* trained in-home exercise volunteers make in-home visits a minimum of
   once a week to teach participants 10 simple, yet progressive exercises
* completion of participant information questionnaire
* participants keep a daily log of their exercise activity

User Fee:
The cost of the Program is $30.00 for 15 in-home visits, this works out to $2.00 per visit.

+/- Special Steps Program

Location of Service Delivery: Perth County

Hours of Service:  Approximately 1- 2 hours per week (based on clients/caregivers wishes and/or needs)

Client Demographics:  Individuals diagnosed with Alzheimer Disease or another Alzheimer-type Dementia and are living independently in the community

Service Components:
* Volunteers receive 6 hours training, plus 4 hours in a VON Adult Day Service in which clients with Alzheimer Disease or another Alzheimer-type Dementia are in attendance.
* Volunteers are matched with clients on a one-to-one basis.
* Volunteers offer support and friendship to the clients through ongoing regularly-scheduled visits/walks.
* The walks can take place in the clients neighbourhood, schools or at local shopping malls.

Transportation:  Transportation of an occasional or social nature may be provided within the ongoing one-to-one relationship.

User Fee:  There is no cost to clients unless transportation is provided.

Location of Service Delivery:  Northern part of Perth County including Listowel, Atwood, Gowanstown, Molesworth and other surrounding rural areas

Client Demographics:
* seniors, chronically ill, or disabled persons living in the community who are lonely or socially isolated

Service Components:
* volunteers matched with clients on a one-to-one basis
* volunteers offer support and friendship through ongoing, regularly scheduled visits
* social visiting in the home
* assistance with grocery shopping
* outings: drives, a meal out, or visits to other friends or family
* walks, cards, crafts, music or other hobbies of interest

Transportation:
* transportation of an occasional or social nature may be provided within the ongoing one-to-one relationship

User Fee:
* no cost to client unless transportation is provided
* client is responsible to pay volunteer directly for mileage expenses 
* the service provides guidelines for reasonable cost for distances travelled

+/- VON Transportation Service
  • Volunteer Drivers
  • Accessible Mobility Bus & Van

Location: Eligible individuals residing in the Municipality of North Perth

Hours of Service:  Based on  the needs of the clients  and the availability of the volunteer drivers for volunteer transportation.  Mobility bus/van services operates Monday to Friday 8:00 am – 5:00 pm.  Trips requested outside of these service hours are accommodated based upon driver availability.  

Client Demographics:
* seniors, disabled adults and needy individuals living in the community who require transportation

Service Components:
* Accessible transportation (Mobility Bus & Van) provided by staff, volunteer drivers, using their own vehicles, provide transportation to seniors, disabled adults and needy individuals, who require transportation to appointments and other activities (in town or out of town)
* referrals are made to other organizations when appropriate

User Fee:   Fees are based upon mileage rate of $.37/km, plus any parking costs.  Accesible Transportation fees range from $4.00(one-way) to $.80/km for out-of-town trips.  Fees are billed monthly
