  Adult Day Programs 

Adult Day Programs are organized day programs of therapeutic activities for older adults and those with physical disabilities who still live at home. 






We have sites in Tillsonburg, Woodstock and Ingersoll. 

People benefit from increased social interaction and stimulation through activities such as exercises, quizzes, games, music, themes, crafts and special celebrations.  Our clients can have access to our bathing program and foot care while visiting the group.  VON provides this program to promote people living as independently as possible for as long as possible in their own homes.  Caregiver relief can be an important aspect of this program.  A hot lunch is served.

Come for a day, no charge and no obligation and see how you like the group!

Programs run from 8:30 am - 4:00 pm.

Early Alzheimer Day
 
The Early Alzheimer Day is for those dealing with the early stages of Alzheimer's disease who would benefit from peer support and recreational and social activities.  Caregiver relief is also an important aspect of this program.  This program is offered every Friday at 570 Ingersoll Ave, Woodstock.

Bathing Program 
The Bathing Program is available to anyone who has difficulty bathing at home.  We have a technologically advanced, easily accessible tub and professional staff.  Together this provides the most advanced cleaning and bathing possible in a comfortable and  relaxed environment.  Bathing Programs are offered at our Woodstock, Ingersoll and Tillsonburg locations.

+/- Blood Pressure Clinics -Free

Blood Pressure Clinics - The VON  Oxford Cardiovascular Health Awareness Program +Action Plan (CHAP+AP)

The CHAP + AP program targets older adults at high risk for future cardiovascular disease (CVD) and stroke and is designed to support prevention of CVD in Oxford County.  These clinics are provided for our community at no cost to the client.

CHAP+AP relies on the participation of family physicians, pharmacists, volunteers, community organizations and older adults.

Older adult residents are invited to attend sessions held at local pharmacies for cardiovascular risk assessment including blood pressure monitoring. Program results are sent to each participant’s family physician and regular pharmacist.

A community health nurse is present at all CHAP+AP BP Clinic Sessions.

Volunteer Peer Health Educators:
 •  Complete a cardiovascular risk profile assessment and Blood pressure monitoring.
•   Offer available community resources (i.e. Pharmacist /Meds Check Program,    Oxford County BP Education Program, VON SMART Exercise Program etc).
•   Provide information for interested participants to improve self-management of modifiable risk factors. 
•   Link participants to the web-based Heart & Stroke Foundation Blood Pressure Action  Plan.

Children's Services 
Children's Services is a family directed respite program. 

This program offers supports that make it easier for people with disabilities to live at home,   serving people of all ages with developmental disabilities and children under 18 with physical disabilities. 
Support workers help people with disabilities to develop new skills and join in community life. The workers also provide caregiver relief and respite so family members can have weekly periods of time for themselves or other family members.

Supports are tailored to each family's specific needs and the best possible matches are made between families and their support workers.

Foot Care Services

Maintaining healthy feet is vital to maintain optimal health, mobility and independence.  As our population continues to age, and aspire to live longer and more active lives, the need for preventative foot care continues to increase.  The elderly, as well as high risk clients with diabetes, arthritis, circulation problems and other health problems benefit from specialized foot care services provided by our caring, professional footcare nurses.  VON Oxford is also proud to be contracted by Veterans Affairs Canada to provide foot care services for our veterans.

  • VON foot care providers are Registered Nurses and Registered Practical Nurses who specialize in foot care and possess advanced knowledge and training.
     
  • VON uses state-of-the-art sterilization equipment called an 'autoclave' -- the most effective form of sterilization possible-- to clean their instruments.
     
  • VON foot care nurses assess your feet and provide preventative foot care suggestions, teaching, and overall foot care -- not just nail care!
     
  • For those unable to attend a clinic, In-Home care is also available.

Friendly Visiting Volunteer Program 

Volunteers provide companionship and share activities with lonely seniors and lonely adults with disabilities. A weekly or bi-monthly visit provides social interaction and improves quality of life.
Visits may involve conversation, sharing an activity in the home, playing cards or board games, going out for coffee, a walk, or other outing.

Referrals can be made through CCAC, family, friends, health professionals, or a person may refer themselves. A Volunteer Coordinator will explain the program in more detail and determine in what ways a volunteer could be of benefit.

+/- Grief Support Group

Grief Support Group
(6 weeks - 2 hrs per week)
Sessions are lead by a nurse/volunteer with experience and knowledge about grief.
                        NOTE:  This is grief support,  not therapy.

Groups are offered in various locations across Oxford county, based on number of requests, and are FREE of charge.
A minimum of 4 people are needed to run a group, (maximum 7 people)
                                   Pre-registration is required 
 Everyone is required to meet with the program coordinator prior to being accepted into a group.

If the person and/or the coordinator feels the group setting will not be a good fit, information will be provided to assist in finding other support or grief therapy.

Volunteer Follow-Up Grief Support

Follow-up support is offered on a 1:1 basis as part of the Hospice Volunteer  Services when a volunteer has an established relationship with the family prior to the death of a family member.

Home Help Program 
A Light housekeeping/errand service for seniors or adults with physical disabilities.

This program offers assistance to maintain a healthy, safe and independent lifestyle.
Services can include any of the following:

  • light housekeeping including vacuuming, dusting, mopping, laundry, cleaning bathrooms and kitchens, etc.
  • local errands including grocery shopping
  • meal preparation.
    Service can also include:
  • one time cleanings
  • occasional cleanings
  • assistance in packing/cleaning home to move to Retirement/Long Term Care Facility

    Compassionate trained staff with background police checks.
    Flexible service hours are available Monday to Friday.
    Free In-Home Assessment.
    Services can be arranged quickly with no contract.

Life After Stroke is a new program supporting stroke survivors transitioning from hospital to home.

Stroke Survivors can be referred by family, physician, the hospital or other community organization.  Or, simply call us yourself.  

 Once you’ve been referred, our Program Coordinator will meet with you to learn about you and your needs.  You will be given information about Community Support Services.

 What follows this first visit depends on your needs.  At your request, the Program Coordinator may refer you to other community agencies that can help you transition from hospital to home.  

A specially trained program volunteer will contact you to support you with your personal goals, including follow up phone calls to see how you are managing.

There is no fee for Life After Stroke.  Our charitable programs are supported by donations from the public.

Meals On Wheels 
Meals on Wheels is a support service that provides clients with well balanced, nutritious meals. Our home-style, cooked meals are designed to meet the nutritional needs of seniors. A variety of entrees, soups and desserts are available and special diets can be accommodated. Traditional hot meals or a supply of frozen meals are delivered by our caring volunteers or can be purchased at VON. The meals are delivered weekdays at noon in Woodstock and Tuesdays and Thursdays in Tavistock. Bi-weekly deliveries are made to some rural areas in Oxford County.

Welcome to VON Oxford Sakura House

Sakura House is an end-of-life care center that feels and functions like a home.  The focus of care at Sakura House is the patient and the people in his/her life circle.

Our private patient rooms include comfortable space for family members to stay day or night.  In addition, the patients and visitors have the opportunity to experience the outdoors on their own private patio or throughout the various gardens and pathways that encircle the property.  Special family spaces, which provide the conveniences and comforts of home, are available to encourage and enable families to take time out while remaining in close proximity to their loved one.

Sakura House is a place where people can feel safe to grieve and know that a caring team of professionals and volunteers will provide support through this difficult time.

The goal at Sakura House is the provision of physical, psychological /emotional, social, spiritual and practical end-of-life care and support which neither postpones nor hastens death.

The Sakura House Care Team continuously strives to employ the most current and highest standards of care and support available. Technology is utilized but kept hidden as much as possible to preserve the home-like feel of the facility. 

Patients can self refer or can be referred by family members, health and social agencies, medical professionals or clergy. The referrals will be coordinated through the CCAC (Community Care Access Centre) to help make the planning and transition as smooth as possible for everyone involved

Complementary Therapies

Reiki, Therapeutic Touch, Music and other therapies are part of many individual’s symptom management regimen.  Several of our volunteers are skilled in these and other therapies and may be able to offer a session as part of patient care at no cost to the patient or family. If a volunteer is not available to offer a particular complementary therapy, patients or families are welcome to make their own arrangements at their own cost.

Note: Aromatherapy may only be offered under carefully controlled conditions due to fragrance sensitivities. Please speak to someone on the care team if you wish to implement aromatherapy.

Discharge from Sakura House
As with all things in life, sometimes circumstances change.  Sometimes the patient improves or stabilizes enough to consider leaving Sakura House for a while. This is cause for celebration! However we recognize that often this is also cause for concern; where and how will care be provided?
 The Sakura House care team will work closely with the patient, family, physician and community care team to develop a best-fit plan including re-admission when needed. 

Leave of AbsencePlease speak to the RN on duty or the Hospice Care Coordinator to arrange for the patient to leave the Sakura House grounds for a few hours.

Donations

Family Comfort
The focus of care at Sakura House is the patient and his/her family.  Each patient’s room includes a recliner and sofa-bed for family use day or night. There are several quiet spaces inside and outside for family members or friends to relax or rest.

Overnight stays: One or two family members are welcome to stay overnight with the  patient as needed.  Please let care team member know before midnight if you plan to stay.

Fire Safety
Although we strive to keep the atmosphere of Sakura House very home-like, we also need to focus on safety at all times. We run fire and other emergency planning drills on a regular basis. We will inform all patients and visitors in advance and will try to keep the drills as unobtrusive as possible.

Flowers and Balloons
Due to allergies please choose flowers with little to no fragrance, and latex-free balloons.

Fragrance Free
Sakura House is a fragrance free zone.  Due to allergies and/or breathing difficulties for many of our patients, please do not wear perfumes, cologne, fragranced hair sprays. etc.

Gardens
All garden areas are barrier free as much as possible. There are some therapeutic garden spaces provided where patients can plant, weed or just dig in the soil as desired. These gardens are accessible from a wheelchair or stretcher. Please ask a care team member for assistance.

Kitchen
The kitchen and dining rooms are considered common rooms and are available for patient and family comfort and convenience. Meals brought from home may be warmed, but full meals may not be prepared in the kitchen.  There is usually a pot of coffee on and some snacks available. A volunteer is on duty in the kitchen from 8 am to 8 pm to assist you. Please ask a care team member for assistance if a volunteer is not present.

Library
The library is situated just inside the front door. Feel free to browse the books and sit and read in this quiet setting. If you would like to borrow a book please speak to a volunteer or care team member member.  This room is delegated as a “quiet space.” (See Quiet Spaces below)

Meals
Patient Meals:  Familiar food made at home is often preferred by our patients and can be warmed in the patient’s microwave or the kitchen microwave. We keep a selection of frozen Meals on Wheels on hand so they can be heated if a patient wishes. Soft and easy to eat snack-foods are kept available for patients or may be supplied by family members to meet special preferences.  

Family Meals: Families are invited to use the kitchen for preparation of snacks only and are responsible for providing and preparing their on food. A kitchen volunteer will provide orientation to the kitchen including a basic overview of basic food preparation, clean up and disposal of food.

Media Room

The media room is considered a multifunctional/common space. Patients, families and friends may use this space as a small group to visit, watch movies, listen to music, etc.   The media room also functions as our children's area.  Dying is a normal part of living and children can experience this more positively when it is approached in a calm and non-threatening manner. The media room can provide a sound-proof, child-friendly zone where children can be children as they cope with a stressful situation through play.

When visiting children need supervision, a volunteer may be arranged. Please ask the Supportive Care Coordinator in advance when possible, for a volunteer to be present for a specified time period.

Patient Rooms
Bathrooms: Just like home, family and friends may use the patient’s private, fully accessible bathroom and shower.

Beds: Our beds and mattresses are specifically designed for added comfort for patients who spend long periods in bed. Your nurse will demonstrate all the various functions and comfort features of the bed.

Call Bell System: Patients are connected to the care team 24/7 via a silent paging system which can be activated from a button the patient is wearing or from the phone set.  

Fridge and Microwave:
Each room has a fridge and microwave for patient or family use.

Phones:
Every room has a phone for patients to use to connect with family, friends or the care team at Sakura House. Due to limited lines to the house, there may be times during increased phone usage when no outside line is available. Cell phone use is acceptable in all areas of the house.

Patient Laundry
Family is requested to take home their family member’s personal laundry. If family members are unable to do this due to distance or other factors, please speak to a member of the care team to make alternate arrangements.

Personal Supplies
Patients or family members are asked to please bring personal hygiene items and toiletries from home as needed.

Pets
Pets are family members to many people but special visiting privileges must be pre-arranged.  Proof of current vaccinations will be required prior to approval for a pet visit.

All pets must be:
• quiet and well behaved,
• clean, free of fleas and odor free,
• accompanied by a responsible family member/friend 
• on a leash no longer than 6 feet or in a carrier at all times while on the hospice property
• escorted into the  patients’ room via the deck to avoid exposure to other patients and visitors who may be allergic to or fearful of animals. 
 
Note: Pet escorts are responsible to “stoop and scoop” -  clean up behind the pet.

Quiet Spaces
There are several designated “quiet spaces” throughout the house. (Library, 2 Sitting- rooms & and the Family Lounge & Solarium). These areas are prioritized for confidential conversations, supportive meetings or a place to rest and regroup. Please feel free to use these areas, but be considerate of others’ needs for privacy.

Smoking
Sakura house is a smoke free home.  Outdoor smoking can be arranged for patients who smoke. Visitors are asked to smoke in designated outdoor spaces only and may not smoke near any doorways or areas that may affect a patient’s ability to breathe.

Spas
There are two spas in Sakura House
Ambulatory Spa: This spa is located on the south end of the house, providing a large shower and a whirlpool bath. Patients who are able to walk with no or minimal assistance may use the ambulatory spa. A family member or care team member may assist if needed. Please ask.

Stretcher Spa: This spa is located near the patient bedrooms and can only be used with the assistance of a care team member. Patients can be transported to the spa by stretcher to enjoy a soothing whirlpool bath in a relaxing setting.

Visiting
Hours: Visitors are welcome between 8:00pm and 8:00 pm daily. Please just ring the doorbell.

After hours: Access to Sakura House after 8 pm may be arranged. Please speak to a care team member.

Children: Children are welcome at Sakura House. Please consider other patients needs for quiet at all times. Young children (under 10) may not be left unattended in any area of Sakura House or the grounds. Please see Media Room or speak to a care team member regarding keeping visiting children occupied and reducing their fear and stress.

Volunteers: An essential part of our team:

Patient and family support
Experienced volunteers with basic Hospice Palliative Care education and additional education in patient care and comfort.
Responsible for working along side RN & PSW as an “extra pair of hands” 
In house daily for 4 hour shifts between 8:00 am & 8:00 pm as needed

Respite
Basic Hospice Palliative Care trained volunteers with additional experience and education in patient care and comfort.
Responsible for providing bedside support of patients during periods of family absence as requested.
On-call

Other Volunteer Roles (Spiritual Support, Greeters, Housekeeping, Kitchen, Grounds-keeping etc)

ALL our volunteers are carefully screened and receive a baseline training relevant to their role. Every area and function at Sakura House depends upon our compassionate and dedicated volunteers. Responsibilities vary depending upon the role the volunteer is filling.

In order to preserve the home-like feel of Sakura House no care team members wear uniforms.

If you wish to speak to a specific person or you are not sure if you need to speak to an RN or volunteer, just ask anyone on the team. We are a small and closely knit team …we’ll redirect you or find the person you need.

SMART Program (Seniors Maintaining Active Roles Together)
In-Home/Group Exercise Program 
 
The VON Oxford SMART Program promotes greater well-being among adults 55+ in Oxford County.  Classes are offered in Woodstock, Ingersoll, Tillsonburg and rural locations throughout the county. This fun and gentle exercise program, provided through group classes or one-on-one in your home: 

• will improve your ability to participate in activities of daily living.

• may prevent health complications associated with aging.

• can help you maintain your independence.

• aids in prevention of falls by improving your confidence and abilities in the  
specific areas of balance, coordination, flexibility, strength and endurance. 

• can help you make new friends

• classes adapted for any seniors who would like to improve their health and well-being.
 

Exercises will strengthen heart muscle, increase lung efficiency, slow the rate of bone loss, maintain elasticity of major blood vessels, and help with weight management and fall prevention.

This program consists of 10 simple yet progressive exercises performed daily within your own home with a trained HSEP volunteer visiting once a week.

Special Services at Home 

Special Services At Home is a family directed respite program, which offers supports that make it easier for people with disabilities to live at home.

We serve people of all ages with developmental disabilities and children under 18 with physical disabilities.

Our support workers help people with disabilities to develop new skills and join in community life. The workers also provide caregiver relief and respite so family members can have weekly periods of time for themselves or other family members. Supports are tailored to each family's specific needs and the best possible matches are made between families and their support workers.

Volunteer Hospice Services 
Volunteer visitors provide emotional and practical support to those with a life-threatening or terminal illness.  Volunteers are carefully screened, specially trained, caring, compassioante and dedicated individuals who offer their knowledge and experience to help both the client and his/her family through the disease process, treatment and outcome of the illness.

Hospice Palliative Care is one of the final and most precious gifts we can offer our loved ones.

Hospice provides:

  • Companionship through home visits
  • Respite care, to give at home caregivers a break
  • Emotional support for all loved ones
  • Telephone support, friendship, sharing and listening
  • Support for coping with change and loss
  • Ongoing bereavement support after the death of a loved one

                    Choice of care is the heart of Hospice Palliative Care.

Referrals are accpeted from individuals and family members, health and social agencies, physicians, nurses, clergy and others.

Workplace Wellness Clinics

Canadians spend over half of their waking hours at work. It is important that workplaces provide a safe environment that promotes overall health and wellbeing.

Often employees are asymptomatic in the early stages of diabetes and heart disease. Our professional staff can come to your workplace and assess your workforce using any or all of the following:

  • blood pressure
  • blood sugar levels
  • blood cholesterol levels


If your employee is found to be at risk, then we can help them on the path to better health, as well as recommend them to a doctor if needed. Together we can decide the best action to improve the health of your workforce through the implementation of healthy workplace initiatives designed specifically for your company.

When: Thursdays

YSS Program Model:

Support/Discussion Group, Guest Speakers
Skill Development and Goal Setting Session
Exercise Program
Hot Lunch in the Adult Day Program
Recreation in Adult Day Program
The YSS participants have the option of leaving after 1/2 day (3hours or less) after the YSS portion OR staying for lunch and recreation in the afternoon at the Adult Day Program.

+/- Youth Volunteer Achievement Certificate Program

Youth Volunteer Achievement Certificate Program 

Youth volunteers visit with lonely seniors and lonely adults with disabilities in their home or in a retirement home. Seniors enjoy the company of young people.

Youth volunteers will share a variety of activities with the person they are matched with. Some activities may include engaging the client in conversation, reading to someone with sight impairment, writing letters, taking them for a walk on foot or in a wheelchair, playing cards or board games, doing a craft, puzzles, occasionally attending a retirement home special event with their “friend.”

A Youth Volunteer Achievement Certificate is awarded after volunteers attend 2 (2 ½ hr training sessions) and complete 20 hours of volunteer visiting. Registration for volunteers is through the Student Services Department at High Schools or by contacting the Volunteer Coordinator at VON Oxford.
